No Show and CancellatioN Policy

 

To best serve all our patients, we kindly request that cancellations be made at least 24 hours in advance. Missed appointments or cancellations made with less than 24 hours’ notice will incur the full service fee. If you arrive more than 10 minutes late, your appointment may be forfeited, and it will be at the clinic’s discretion whether we can accommodate you or if the visit will be considered a no-show, resulting in a full charge. We appreciate your understanding and cooperation in helping us maintain efficient and timely care for all our patients.


DOT Policies

To comply with FMCSA regulations, all patients arriving for a Department of Transportation (DOT) physical must bring a valid driver’s license, a complete list of current medications (including dosages), and any corrective lenses (glasses, contacts) or hearing aids they use. Failure to provide these items may result in rescheduling your appointment.

Fit Test Policy

 

Self Pay Policy


All fit test candidates MUST be clean shaven to successfully complete the fit test to OSHA standards. Facial hair such as beards, goatees, and mustaches prevent the mask from fitting tightly to the face and makes it impossible to achieve a perfect seal. This leaves the employee vulnerable to whatever particulate matter they are wearing the mask/respirator to avoid. Candidates who arrive to their appointment with facial hair will be asked to shave and reschedule.


By choosing self-pay drug testing, you acknowledge that payment is due in full at the time of service and is non-refundable. It is solely your responsibility to confirm with your employer or requesting party that they will accept a self-paid test from our clinic before you proceed. We cannot guarantee acceptance, as many employers have specific vendor or billing requirements we may not meet. By paying for this test, you accept all responsibility if your results are rejected, and no refund will be issued.
